Phoenix Inferno Performance Lab — Participation, Assumption of Risk, Release & Acknowledgments

I understand that participation in strength and conditioning, speed and agility training, plyometrics, conditioning, testing, mobility work, athletic development activities, and use of training equipment involves physical exertion and inherent risks. These risks can include falls, collisions, strains, sprains, fractures, equipment-related injury, cardiovascular events, and other serious injury, including risks that cannot be completely eliminated even when reasonable coaching and safety practices are used.

I confirm that the athlete is physically able to participate to the best of my knowledge. I agree to disclose relevant restrictions, injuries, physician-imposed limitations, or other information that should affect training. I understand the athlete must follow coaching instructions, use equipment as directed, and stop and report pain, dizziness, unusual shortness of breath, or other concerning symptoms.

I voluntarily accept the ordinary and inherent risks of participation. To the fullest extent permitted by applicable law, I release and hold harmless Phoenix Inferno Performance Lab, Phoenix Inferno, their owners, coaches, staff, contractors, volunteers, facility partners, and affiliated organizations from claims arising from the ordinary risks of participation or ordinary negligence. This release does not apply to conduct that cannot legally be released or waived.

If urgent care is reasonably needed and I cannot be reached, I authorize staff to obtain emergency medical assistance for the athlete. I understand that I remain responsible for medical costs not otherwise covered.

I understand that this electronic acknowledgment and typed signature are intended to serve as my signature for this waiver. I have had the opportunity to read the waiver before signing and may ask the Performance Coach for a copy.

I have read and agree to the participation waiver above.I understand the risks, responsibilities, release, and emergency-care authorization described above. I acknowledge that photos and videos may be taken during Performance Lab activities.I understand that the athlete may incidentally appear in training, team, facility, or event photos/video. This acknowledgment records awareness that photography/video occurs; it is not a separate blanket grant of promotional-use rights.
